Medical management plan templates
Our medical conditions policy was recently updated and it’s aim is as follows:
The service and all educators can effectively respond to and manage medical conditions - including asthma, diabetes and anaphylaxis - to ensure the safety and wellbeing of children, staff, volunteers, students and families.
One way that we can achieve this outcome is by families and health care providers utilising medical management or action plans that tick all the required boxes for us. That way we will know exactly how to respond in an emergency.
There are important details that are often missed on a medical management plan and medication, including:
A photo of the child
What triggers the allergy or medical condition
What first aid is needed
Contact details of the doctor who signed the plan
Both the dates of when the plan is to be implemented and the date for review
Relevant supporting documentation
A pharmaceutical label with the child’s name, expiry date, and administration instructions on medication or packaging
Please use the drop down menus to download a medical management plan template that we recommend. You can also refer this page to your health care provider for their reference.
